I placed each generation into different files to keep download time to a minimum:
These forms are in Microsoft Word (ver 6.0 for Windows95), and are simply word documents with tables, which will allow you to print out the form and mail it to me, or fill it out online and send it back to me via e-mail. Eventually, I would like to have an online area to allow you to input your information directly over the web, but that will take a little more time.
Welcome to the Sibling Profile page. This page allows you to provide information for 1 brother/sister. It also asks you for your information on your brother/sister’s children, and their children as well. If you have more than one brother/sister, then feel free to cut and paste and include your other siblings in this file, or complete one at a time and e-mail or mail them to me separately.
In addition to filling out this form, please feel free to send any pictures, documents, newspaper clippings, or other interesting Stubblebine information to me If requested, I will send this information back to you or pay you to make copies for me. My address is listed at the end of this letter. I will scan all of this information and collect it for all to view out on our web site at www.stubblebine.com/family. If you should need additional forms, please visit my site to download these forms, or call me, and I will send them in the mail to you!
1. Fill out your name, Date, and Sibling number x of x fields. Phone number is optional, but it would be useful if I need to contact you for clarification.
2. Fill in all of the information in the colored boxes. Please include as much information as possible. This form allows up to four children for each generation; if you have additional information, cut and paste the form to create the additional area. If you do not feel comfortable with disclosing information in certain fields, write "private" in that box.
2. Fill in the numbers in the colored boxes on the left hand side of the page.
For example...John Stubblebine is your brother and married Mary Smith, and had three children, Jack, John and Alice. Jack was married to Barb, and had two children, Allison and Betty. John Jr. never got married, and Alice was married to Bob Ackerman, and had two children, Jill, and Joyce:

If this was the profile for your #2 sibling, you would merely change the numbers to be 2.x.x. 3. Save your sibling files as siblings1.doc, siblings2.doc, siblings3.doc, etc., If you have more than one sibling, you can enclose each sibling profile sheet within the same file. I have not made an accomodation for divorce yet on the Siblings Form, so just insert your information where neccesary if you have a case of a divorce in your lineage.
Don’t wait until you have all 5 of your brothers and sisters done at once! Take your time, and send the sheets to me as you get them completed! As I receive these sheets from you, I will add them to my database and update the web site, so check back often to watch the Family Tree grow! If you’re typing information directly into the form, and you misalign the tables, don’t worry about it. I’m going to extract this information anyway, and the form only serves as a template to get your information in a neat and organized manner.
I understand that this is a lot of information to fill out, and may seem like a cumbersome task. However, if we all work together, the little time we take now will hopefully reap immeasurable returns later on!
